Fairfield High School will be having their Sports Physicals Day on Wednesday, May 15, 2024, from 5:00-7:00 p.m. at Freestone Health Clinic.
The cost is $25, payable by cash or check (no insurance).
Please bring completed sports physical form signed by parent or gauardian.
